After more than 30 years of practice, I will be retiring and closing my orthopaedic medical office as of July 31, 2025. The last day I will be seeing patients is July 18, 2025.

 

If you wish to request a copy of your medical records, you may request them from this office through July 22, 2025. 
-In accordance with applicable state law, you will be charged for the copying and the transmittal of the records. Cash payment  ($0.50 per page for patients under workers compensation) is due at the time the records are picked up or delivered. 
-Please note that July 22, 2025 is the last day records may be picked up or sent from this office.

 

As of July 23, 2025, you may submit a request to the Records Custodian:

-Please note:  There may be a short transition period between the office closure and the time Morgan Records Management has full access to my charts. (They will tell you the fee for the medical records.)
-In accordance with Florida law, medical records will be retained for at least five (5) years from the date of last treatment.
